INDEMNITY AND RELEASE:
PARENTS / LEGAL GUARDIANS
We the above Parents/ Legal Guardians are the parents or legal guardians of the above Applicant (“our child”). We agree to our child applying to and being allowed to participate in cricket matches, training and associated events (“Activities”) organised and or supervised by Herbert River Cricket Association (Juniors) or its Affiliates (“Association”). In consideration of the Association allowing our child to take part in the Activities, we acknowledge, agree and confirm the following:
(a) That there are risks associated with the Activities which may result in our child being injured including in a serious manner. We fully accept and agree to bear those risks in our own right and on behalf of our child
(b) to the full extent permitted by law we agree both on behalf of our child and in our own respective rights to absolve, indemnify, release and discharge the Association, its officers, employees, representatives and agents (“Indemnitees”) from any and all liability for any injury, loss, cost, expense or damage suffered by us or our child however caused, arising from or incurred directly or indirectly as a result of our child’s participation in the Activities, including without limitation as a result of any act, omission, default or negligence of the indemnitees.
CONSENT TO DISCLOSURE
PRIVACY ACT: PARENTS / LEGAL GUARDIANS
We the above Parents/Legal Guardians, as the parents or legal guardians of the said Applicant, above, (“our child”) herby agree and consent to the provision of the personal information regarding our child as set out in this form to the Queensland Cricket Association Limited for use by it as it sees fit in the course of its administration of cricket in Queensland.
EMERGENCY MEDICAL TREATMENT
We the Parents / Legal Guardians of the above Applicant authorise the Association to provide emergency treatment of an injury to or illness of our child if qualified medical personnel consider treatment necessary and perform the treatment. This authorisation is granted only if we cannot be contacted and reasonable effort has been made to contact us. We agree to accept any and all costs of emergency treatment provided to our child.
ASSOCIATION MEMBERSHIP
One Parent/Legal Guardian of each player or from each family, will be nominated as a Member (at no cost) of Herbert river Cricket Inc. if acceptance of the nomination is indicated below. See our helpful committee members for more information regarding HRCA membership.
